The facility assessment, the QAPI program, the improvement projects, the survey process itself — every one of them is written as something a building does, continuously. Most facilities run them as documents instead: assembled before a survey, filed after, and rediscovered the week something goes wrong.

The distance between what a facility actually does and what it can show is where citations come from. We build for the program and let the document fall out of it.

Mock Survey

Practice the CMS Long-Term Care Survey Process end to end — offsite preparation, initial pool, triage, investigations, wrap-up. All 44 CMS care-area pathways, MDS-driven resident selection, and a survey-ready binder with deficiency narratives, plans of correction, and a record of who did what.

Facility Assessment

42 CFR §483.71 treated as a continuous staffing and resource monitor rather than an annual form. MDS prefill with deltas against your baseline, per-topic sufficiency with surveyor-focused guidance, copy-forward refreshes, and governance sign-off.

QAPI Plan

A builder for the §483.75 program plan: QA&A committee composition checked against the mandated seats, governing body accountability, performance indicators with named owners and review cadence, adverse-event and near-miss tracking, and export mapped to F865, F867, F868, and F841.

Improvement Projects

A real QAPI loop rather than a narrative form. Charter, root-cause analysis, interventions as first-class records with PDSA cycles, process and outcome measures, meetings that generate their own tasks and decisions, then conclusion and a sustainment plan.

QAPI Self-Assessment

The CMS self-assessment tool — 24 statements across five domains on the 1–5 maturity scale — scored by domain and overall, with low ratings routed straight into action planning and compared year over year.

MDS pipeline & dashboards

Upload your submission files as they come. Per-facility routing, correction and inactivation matching, duplicate detection, and safe resident identity checks — then a dashboard per module, and a roll-up across buildings for multi-facility operators.

In the roadmap: emergency preparedness, life safety code, action indicators, and the satisfaction line — CoreQ, resident, and family.

How we build it

Three commitments that shape every screen.

Your data, not a blank page.

The system reads the MDS you already submit and fills in what it can. The job in front of the coordinator is reviewing and affirming what the data says — not generating a document from nothing at eleven at night.

Everything is evidence.

Every artifact carries who did it, when, and why. Contributors keep their attribution after they leave the building, so a surveyor asking who signed off in March gets the right answer instead of a blank.

AI at design time, never at render time.

Where we use AI, it edits a document you can read and change. It does not sit between you and your numbers, and a page it helped build renders the same way next year whether or not you ever use it again.

A tool, not your compliance officer.

We do not promise a survey outcome or a star rating. No software can, and a vendor who implies otherwise is selling something no surveyor will accept. What we sell is a system of record for the work — one that makes what your facility is already doing legible, current, and provable.

Security & privacy

MDS data is PHI. We treat it that way.

Quality & Compliance Systems operates as a Business Associate and executes a BAA before any real resident data is touched. That is a precondition of the first demo, not a procurement afterthought.

Production data never leaves production. Development and testing run on synthetic assessments built for the purpose — never on a copy of your residents.

Quality & Compliance Systems is a Colorado limited liability company formed in 2026, built with long-term-care clinical expertise on the team rather than consulted after the fact. Every product decision on this page traces back to someone who has sat through the survey being modeled.

Pricing is per module, month to month. You turn on what you use, you pay for that, and you can leave. Compliance software that needs a three-year lock-in to keep a customer has told you something about itself.
